Open Kuvempu University web page, Take a DD and send it to University. While sending DD, Write a letter to Registrar and Finance officer with your register number and for what purpose you are required n send them. Check with the University about whether they received your application or not and wait for sometime, they send u directly with the addresses which you provided in the application.
