As far as the US goes, the institution to which you apply in the US will correspond and communicate with your former institution. They will check their accreditation status and will check the ranking/registration of the association that holds the accreditation of your institution. Then, they will evaluate your transcripts in comparison with graduates who have also went to your same school. The US gets a lot of bogus documentation, so they are used to taking as much time as necessary to validate and confirm not only your transcripts, but also the existence of the school and its true standing in the education universe. For example, IIM Bangalore will have pristine processes and documentation. Some schools, like the Asian College of Teachers cannot be verified, so students who study at these kinds of schools cannot obtain admission into the US system using their records at all.
